1.  By 123FormBuilder

    123FormBuilder (previously 123 ContactFormBuilder) is a cloud tool to build any different types of online forms, manage workflows, and collect their data very quickly. 

    It works in any CMS platform, including BlogSpot and WordPress.

     Step 1 - Create a Free Account by going to 123FormBuilder.

 

     Step 2 - Login and Click on the Dashboard.

            

      And then click on the New Form button to create a New Form.

     On the second page, click on the Newsletter Subscription Form. A Contact Form Will Appear On The Second Page Which Is Ready To Be Used. 

  You can add any extra fields you may want by clicking the fields button.

     

     Click on save and publish button.

     Step - 3: Click on Embedded Button

 

   

 Step 4 - Copy the javascript code from the screen. (Press CTRL + C) or click on the                        Copy code button.

 

    

    Step 5 - Go to your Blogger dashboard and click on add new page. Name this page                      as "Contact Us. "

 

   

     Step 6 - Click on the HTML format editor button and paste the code here and click to                     publish.

                Your contact form is now live on your blog. Take a look at it.
